Should I Get Cerclage Done Sooner In Order To Stay At Home During Pregnancy?

I know I am going to need a cerclage, as I nearly lost my last baby at 17 weeks, and Had to have a cerclage and Medical bedrest. What will this cerclage entail this time around? How long will I need to be in the hospital? Will I need someone with me? Also, I'm told the sooner I get the cerclage, the better chance I have at staying home for this pregnancy. Would you agree? My daughter came at 25 weeks so the Dr did a T cut on my uterus. I'm told I scar very well, but is uterine rupture a danger? THANK-YOU for answering my questions. While we are really excited, the last experience (6 months total living in the hospital! including 4 mos in the NICU) has me reeling over what could happen this go round.

OBGYN 's  Response
Hello, I would be happy to help you with your question.

This is a very complicated situation and question and is more appropriately submitted as a Premium question to OB/GYN specifically. Here are some general guidelines for patients in similar situations:
1. It sounds like a cerclage is indicated
2. It is ideally placed right around 13-14 weeks (at 17 weeks it was less ideal)
3. You will need weekly ultrasound checks of your cervix - this will determine if you are home or hospital bound
4. Based on the incision on the uterus, a C-section is a given and would usually be timed at about 37 weeks (if you make it)
5. The risk of uterine rupture is high IF you labor (approaches 5-10%), and thus the reason for the earlier C-section scheduling

If you would like additional help, please pose a Direct Question to me, Dr. Raichle and I would be happy to help you sort this out further.

I hope that this helps and good luck!
