Spinal Stenosis, Congestive Heart Failure, Atrial Fib, Osteoarthritis. How To Control Pain?

A friend has spinal stenosis. She is 87. She also has congestive heart failure, atrial fib and and severe osteoarthritis. She was helped greatly by gabapentin for about a month but has to stop it now because of abnormal kidney function lab results. She is in agony. Her doctor is away until Nov 19. She also takes Celebrex for the osteo arthrits. It's in a high enough dose to act as an anticoagulant (for the atrial fib). Any ideas for pain control? She cannot tolerate opioids (becomes very depressed).

Dear Friend.
Nice to see your concern for your friend.

She has multiple problems.

As you said that she also developed kidney dysfunction, that you are supposing to be because of gabapentin. But i must bring into your notice, that she is taking Celebrex(celecoxib), which is an Non Steroidal Anti inflammatory Drug (NSAIDs) , which causes renal failure and also causes increase cardiovascular problems and mortality.
And to add to your knwldge, it is not an anti coagulant.
Moreover, since NSAIDs also leads to peptic ulcers and upper gastro intestinal bleed, she is on high risk for that with serious consequences as she must be recieving anti coagulant.
My advise is to STOP Celebrex immediately.

Now the pain part is difficult to manage as she can't take NSAIDs and she feels depressive with opiods.
She can try capsaicin gel locally.
If she has worse pain , she can apply topical NSAIDs like volini gel.. as they are comparitively less harmful.
There is a drug called as Flupirtine, which is a centrally acting non - opiod analgesic. However not totally safe in renal impairment, but still can be used.
