Hello Shauna,
Welcome to Healthcare Magic and thanks for your query.

I understand that you are going through a very difficult time due to you multiple psychiatric problems such as depression, OCD and Borderline Personality Disorder. Effexor (Venlafaxine) is an anti-depressant medication and 225mg is a good dose. Olanzapine is an anti-psychotic medication.

If this combination of medication is not working for you, then you can discuss with your doctor regarding other medication which can be tried. For example, SSRIs can be effective for not only for depression but also for OCD. Certain SSRIs like Fluoxetine are also recommended in the treatment of Borderline personality disorder. In addition, regular and intensive psychotherapy is also essential to give effective results. If your symptoms are very distressing or disabling, then you can consider in-patient treatment for a short period.

ECT has not been proven to be an effective treatment option for intrusive (obsessive) thoughts and hence may not be advisable.

Wish you all the best.
