Suggest Appropriate Dosage Of Acitrom

i am taking acitrom since last one month starting with 4 mg but the INR is not reaching '2' , presently the dose is 6 mg and the INR is 1.58 , I am also taking Heparin 60 mg twice daily How long will it take to control the INR and what is the maximum dose of acitrom a person can take
