I have occasional pain around my left teste, a dull ache that lasts for a few minutes or hours. On self-examination I can find nothing untoward, except a small lump, the size of a small ball bearing (about 3mm acoss), that seems to be completely mobile and tender when palpitated. I think it may be a scrotal calculus. Is there any treatment, or are they completely benign?

Small bump can cause the pain only if it is a cyst that has got twisted. I would advise you the following: Color Doppler with ultrasonography. Clinical evaluation and physical examination by an Urologist.
Surgery for removal of the lump if it is thought to be the cause of pain in the left testis. Medical treatment if it is due to infection like epididymitis and / or Orchitis Scrotal support Evaluation of the Genito-urinary tract by ultrasonography of the abdomen, tests of blood and urine. Further tests of IVP if required.
