As per your complain 
Pilonidal sinus is a small cyst or an 
abscess that occurs at the top of the buttcrack and at the lower end of tailbone..
In active and infected stage it is highly painful and can interfere with sitting and moving..
It can occur due to a number of causes like:
-Traumatic 
injury to the lower portion of tailbone or top of buttcrack.
-It can occur due to ingrown hairs.
-Long sitting hours.
-Low Immunity.
-Rubbing with clothes..
As you have already been treated with 
homeopathy and allopathic with  no definitive relief, , the last resort left is surgical 
drainage..
You can however be given antibiotics and anti inflammatory painkillers to reduce infection and heal the sinus..
But in case if it still does not resolve then surgical drainage of the lesion followed by complete removal of pus, ingrown hair [if present ] and thorough debridement of the wound and doing antiseptic dressing will be done..
Regular change of dressings and antibiotic coverage will also be advised for faster and better recovery..
