Hi,

Continuous heavy menstrual flow is called as menorrhagia, can occur due to different causes like:
- Hormonal imbalance, that can be seen in 40s.
- Structural causes like fibroids, adenomyosis, polyp etc.
- Ovarian causes like poly cystic ovary disease, ovarian tumors
- General causes like thyroid hormonal imbalance, diabetes etc.

As you are having the bleeding since three weeks and not getting relieved with hormonal pills, better to get evaluated. Consult your Gynecologist once, get examined and go for investigations like hemoglobin levels, blood sugar levels, thyroid profile, ultrasound etc.
With this workup the possible cause of your problem can be identified and treatment can be taken accordingly.

In case of hormonal imbalance anti fibrinolytic drugs and high dose hormonal pills can control the bleeding. Structural causes may need surgical intervention. Though sometimes the pills can lead to tiredness, in your case it could be due to the blood loss you are having. Taking iron supplements along with the prescribed medicines can reduce the symptoms to some extent.

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Sree Gouri S. R., OBGYN
