Suggest Remedies For GERD Relapse, Causing Difficulty And Pain While Swallowing Food

Over the years I have had problems with IBS/GERD/bleeding ulcers. Much has improved. I was not found to have hpylori bacteria. However, I have often been able to feel food move down my esophagus. Lately I have had GERD again, belching, and frequently cannot swallow....particularly if it is meat. I swallow and it gets stuck in between my breasts. It is very painful, and the food will not move up or down. Frequently I vomit trying to get it to move. Tonight my husband had to do the Heimlich on me. This is very painful, and I do have some pain in my upper back between my shoulders. I did manage to get what little food I did not vomit to move by drinking something hot. I would imagine I need to go back to GI medicine to see what is going on....I am beginning to get worried. It is happening more and more frequently,

Hi,
I have read your question and I understand your concern.
The symptoms that you prescribe lead me to think about Esophagitis. It is an inflammatory disease of the esophagus caused by long-term exposition to high gastric acid output. It is associated with pain in swallowing, difficulty in swallowing and GERD. This is a serious condition that needs to be addressed as soon as possible because it can lead to Barret's Esophagus, which is a malignant tumor of the Esophagus caused by the chronic burning of the esophageal cells. You have to visit your GI and do a endoscopy and biopsy of the affected tissue. This is your first step.
Next, you have to work out with your doctor what is the cause of the persistent high gastric acid output. It seems like your GERD is very resistent to treatment, and this can have different causes. I will mention some that seem most probable in your case:
1. Depression, anxiety and other psychological stress can affect the production of your gastric acid and cause a chronic GERD/Gastritis?Ulcers.
2. Smoking and drinking. They have the same effect on gastric acid.
3. Zollinger-Ellison Syndrome. This is caused by a tumor producing gastin, which is the hormone that induces the production of gastric acid. It is resistent or mildly affected by PPi and other GERD drugs, and is often associated with heartburn, IBS and ulcers leading to Esophagitis. Your doctor will have to measure your Gastrin level and acid output levels to diagnose this condition. It can be confirmed by an endoscopic ultrasound (ERCP). I would also chech Calcium levels in your blood to exclude a MEN1 syndrome, often associated with Zollinger-Ellison.

Hope I have answered your question. If you have any further questions I will be happy to help.
Wish you good health,
Eni Manoku M.D.
