Thank you for your query.
Would you please clarify a few things for me?

1)how old are you? Male/female?
2)are you a diabetic, hypertensive or hyperlipidemic?
3)do you smoke or consume alcohol on a regular basis?
4)do you have any history of urinary tract infections or stones in the past?
5)did you have just the blood in the urine for 5years or the pain as well, or did the pain come and go during this period?
6)how severe is your pain on a scale of 1-10 (1 being the lowest and 10 excruciating pain). Please mention the number for the pain 5 years ago & now.
7)what color is the blood in the urine? How do you know that there was protein In the urine?

8)do you have any other health issues or allergies?

When you had this problem the last time, were you investigated for any circulatory problems in the kidney and for the kidney? Was it just for stones or did they check for anything else? What relieved the pain for you and what was your diagnosis?

Kidneys have several structures that make it functional. Nephrons are the functional buildings blocks which have an entry and exit station (entry station where the blood gets filtered through the glomeruli (a bundle of vessels with selective and specific filters) to form urine which exits through the collecting ducts). The Glomeruli are arranged into bigger bundles which serve as. Independent functional units for urine production.

All this urine is then collected into bigger tunes and sent through the ureter and into the bladder.
The kidney also has tissues interspersed between these function units which provide support and serves other functions. Each kidney is surrounded by a capsule which completes it.

Things that can go wrong to cause your symptoms would be either with the Nephrons, specifically the entry point where the lining could be too thin or the filters don't function properly allowing blood to seep through. This can cause focal scarring and damage which leads to pain.

Any obstruction in the form of stones, tumors or infections need to be ruled out.

Any lesion within the kidney which puts pressure on the surrounding tissue and capsule can result in pain and bleeding.

Compromised blood supply to the kidney due to blockage or compression by surrounding structures can cause pain

I recommend urine analysis, renal arteriography (to check blood supply to the kidney), renal biopsy (to rule out any structural abnormalities at microscopic level) and imaging studies (x-ray, CT, mri) to rule out obstructive causes and mass lesions. Blood investigation to rule out anemia(bleeding for 5years is sure to cause it) and to check your other organ functions.

Please go equipped with all the information I requested along with your previous reports. Insist on having them answer your queries. (write down all you need to ask- as in why this happens, what causes it, what treats it, what needs to be done from your side, if surgery is indicated and chances of recurrence, treatment plan for pain, what to expect short term and long term?

5 years is a very long to suffer without knowing what it is. Remember that it is your health which is at stake. You have a right to information and treatment. If they refuse again, find another physician and get a note from your old one stating all that was done for you. You are entitled to a second, even third opinion. Also look for a Nephrologist who you can meet directly.

The mainstay of treatment would be to relieve pain, stop or reduce bleeding, correct anemia, and prevent progression of disease to avoid kidney failure.
I suggest a physician /Nephrologist opinion ASAP.
Hope I've helped. Please get back to us if you need further clarification.
Wish you good health
