Hi,Dear,Welcome with your query to HCM.
Studied your query in full depth of its details.Reviewed it in context of your health concerns.
Based On the facts, You mostly seem to suffer from-
Right Retractile testes in the infundibuliform sac of the cord in the scrotum.
This could be due to the non-fixation of the cremastor and excessive mobilisation of the infundibular sac around the cord along with the right testes which could happen after 3 yrs post-hernia repair,by which time the planar dissection is lined out with interfascial tissue laminar discretions after the post surgical fibrosis.
This might happen due to the non-reposition of the testes in the scrotal sac after the hernia repair surgery.
Treatment -
Would be to assess the testes retracted position with USG studies of right funicular and scrotal regions.
After proper assessment of the defects in this funicular cord sac till the testes,Testiculorrhaphy -ie fixation of the cord cremaster to the surrounding fixation stitches with relocation and fixation of the retracted testes in the scrotal pouch, would be needed.

Procedure similar to herniorrhaphy below external inguinal ring is needed as surgical correction in this case.

Hope that ,This reply would help you to plan further treatment soon with your treating doctors.Best of Luck and early recovery.
Welcome any further query in this regard, which would be replied in next
session.
Good Day!!
