You have history of lupus, Sjogren's and diabetes. Presently you have kidney infection and are experiencing pain under the rib and back. You are suggested to get clinically examined by a urologist, get urinalysis (R/E and C/S) done, stay well hydrated. Get all renal function and imaging tests done. You will be given conservative treatment. Acute kidney failure may cause symptoms like pain on both sides under the rib cage. Yes it may be the cause of backpain felt at a higher level.
To get relieve from discomfort and pain you are suggested to take acetaminophen or naproxen, one stat when needed. Stay well hydrated. Get treated at the earliest. Control your blood glucose levels at all cost. Include anti inflammatory food in your regular diet. Do regular exercise, walking and switch to healthy living. Stay away from addictions if any.
Take care. Hope I have answered your question. Let me know if I can assist you further.
