Understanding your concern. As per your query you have painful, swollen knee due to wear and tear of cartilage. Well there can be many reasons for symptoms you mention in query like forcefully twist or rotate your knee, such as aggressive pivoting or sudden stops and turns. Even kneeling, deep squatting or lifting something heavy can sometimes lead to a torn meniscus. I would suggest you to consult orthopedic surgeon for proper examination . Doctor may order X-rays , ultrasound , magnetic resonance imaging . Doctor may also examine knee with arthoscope . Doctor recommend physical therapy along with rest and icing of knee . If this doesn't work doctor may recommend surgery. If the tear can't be repaired, the meniscus may be surgically trimmed.
Hope your concern has been resolved.

Best Wishes, Dr. Harry Maheshwari
