Suggest Remedy For Penis Flowing From Different Hole

Dear sir, my son is 3 year old he gone under hypospadias surgery for 3 times till now but last surgery happened on 3/03/09 after two months from the surgery again the urine is passing from another new hole which is just beside from the tip may i know why it is happening sir please
