Suggest Remedy For Soreness In Shoulder

Hi. I am 21 and I fell off of a running horse a week ago and landed on my shoulder and hip. My shoulder has gotten better pain wise except when I try to move it certain directions or try to lift something heavy. When it does hurt, I either can't move it or I feel a sharp pain beginning about the middle of the top of my shoulder up my neck. Is this normal soreness or is something wrong?
