Suggest Remedy Pain In Top And Side Of Big Toe

My age is 46, weight 150lbs. No health problems. I am having extreme pain in the top and side along my instep next to big toe. There is a nerve on top of my foot that I can not touch. It send shooting pains from the center of my foot to the tips of my toes. I exercise quite a bit and this seems to have occurred in the past couple of months. It is not getting any better. I'm not even sure what type of doctor to go to. Please let me know what you think.
