VDRL and RPR positive whereas TPHA negative means that you are not having Syphilis at all. This is called as biological false positive. The reagents used in both VDRL and RPR are not derived from the causative agent of Syphilis. So it may be reactive in conditions other than Syphilis also, usually in low titre less than 8.In TPHA test, antigen is derived from the Syphilis causing organism, so this reactive serology is more confirmatory in case of diagnosis of Syphilis. All penicillin were not useful in Syphilis. The exact type of penicillin in the correct dosage for correct duration is essential for Syphilis. Even with correct management, Syphilitic serology may take 6 months or more to become nonreactive. In your case, look for the cause of biological positive reaction and try to treat the same. Consult a physician who is well-versed in the management of Syphilis and follow his advice. DR S.Murugan
