Psychiatrist 's  Response
Hello thanks for asking from HCM

You have depression since childhood and are taking Sertraline 200 mg per day. You have received ECT also and it caused improvement about 10 years back. As you are not getting relief with Sertraline even with high doses so I would advise you to consult a psychiatrist to add some more drug as augmentation agent to your therapy. If a patient fails to respond to one drug then for resistant depression augmentation drugs are used. Drugs as Lithium carbonate, Lamotrigine, bupropion, mirtazepine when added along with sertraline will cause improvement of symptoms. You can visit your psychiatrist for prescription of these drugs.

Along with this attend Cognitive Therapy sessions which will help to reduce your symptoms. If symptoms remain persistent even after that then ECT is treatment modality that should be given attention.

Thanks, hope this helps you.
