General Surgeon 's  Response
Hi,

Noted the history of eczema if ears, throbbing pain in ear and so on.
All the history is indicating toward the itching in ear causing injury.
The injury causing infection in the external ear canal wherein the skin is strongly attached to the bone hence even a small edema causes severe pain.
The edema is so much that there is pain even on touch.
Another cause can be granuloma formation.
Additional fungal infection can be the cause of pain.
Hence I would advise you the following:
Consult an ENT Surgeon for ear endoscopy to know the exact cause.
Get started with proper prescription based antibiotic, anti-inflammatory and topical anti-fungal.
Specific painkiller is the necessity to control pain till the medicines effectively controls the disease.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, ENT & General Surgeon
