Suggest Treatment For A Rash On The Penis Head

I have noticed a rash and a tightening of the skin on my penis just below the head. I have hypospadias so I have a foreskin. When I get an erection the tightening of the skin is worse than when flaccid. When I am flaccid my penis retracts and you cannot tell I have a penis. I have been washing very thoroughly and applying Vaseline which helps getting my penis to come past the foreskin. What, if anything, can I use to clear up this rash and tight skin? This has been ongoing for almost 3 weeks.

It can be due to infection, for now you can go for local antibiotics. But you need surgical intervention and make your penis structurally right for proper sex. Share a picture for confirmation.
