Hi,
He might be having blood vomiting from stomach and food pipe due to increased pressure caused by shrunken liver called liver
cirrhosis. It's a very serious condition and needs
liver transplant for proper management but at this moment bleeding needs to be stopped.
One way is to do upper Gastrointestinal tract endoscopy or OGD along with banding of blood vessels which are bleeding and side by side
blood transfusion to cope with lost blood.
But due to
heart attack and fluid accumulation in lungs due to hearts getting weak to pump blood forward rather dumping it backwards leads to
pulmonary edema or what we call water in lungs usually.
In such condition blood thinners can't be given to open heart vessels as in order to do so, the gastric tract bleeding can get worst. Blood transfusion can cause more fluid in lungs and
difficulty breathing. So it's a very tough situation to be into.
Chain of management should be doing upper GI endoscopy, band ligation, stopping bleeding, alongside diuretics to remove fluid from lungs if blood isn't very low and ionotropic support for a while to increase the BP and try to ease his heart out supportively as
angiography can also be difficult immediately.
Once fluid is reduced from lungs , bleeding stops, BP builds up, blood transfusion to cope with the lost blood, slowly, only way to hope for any chance of recovery after that.
