General Surgeon 's  Response
Hi,

As per my surgical experience, the severe pain not responding to analgesic indicates ligament (sprain, strain or tear) or joint space related issue (arthritis, foreign body in joint space, lipping of articular surfaces or others).

In a nut shell, the issue should not be neglected for home management now, and a consultation with orthopedic surgeon is recommended for further work up with X-ray and MRI of the knee.

Hope I have answered your query. Let me know if I can assist you further.
