Suggest Treatment For Celiac Disease, Stomach Pain, Diarrhea And IBS

Hello. I have my gallbladder taken out a week ago today because of pain attacks I have had over the past years under my right rib. Before surgery I was hardly able to eat. Always getting sick. And always in pain. I also have celiac disease and IBS. So it has been hard for doctors to pinpoint which symptoms go where. I have been gluten free for some time. And after my surgery I was able to eat normal again!! (Minus the gluten). That lasted three days. I went to the ER on the fourth day with incredible stomach pain, diarrhea, and vomiting. I was falling out of my chair in the waiting room in pain. They did blood work and an x-Ray. They said all they found was that my liver enzym levels were high. They called my surgeon and he said that it is normal after gallbladder surgery. The ER doctor told me it was probably a stomach bug and sent me home. Over the next few days I have began having stomach cramps all day, every day. I ve been constipated most of the time with random bursts of terrible diarrhea. I ve heard that s normal. I have had an abnormal thirst that I can not quench. I have drank SO much water and I still feel so dehydrated. And I m not peeing anymore than I normally do. Then, today I had finally started having solid stools. At first they were covered in white. And then it was mixed with clumps of dark red blood. What is happening?? This can not be normal! Any suggestions?

General Surgeon 's  Response
Hello,

* As per my clinical experience, the alternate constipation with diarrhea do indicate flare up of underlying inflammatory bowel disease most likely.

* Apart from gluten free diet, in my clinic, I do recommend

- basic lab works with

@ complete hemogram

@ stool microscopy

@ ESR & CRP

@ ultrasound of the abdomen & pelvis

@ liver function tests

@ colonoscopy

* Depending upon the reports of above , certain management guidelines can be figured out

- drink more liquids as boiled water, lemon juice, oral rehydration salt

- anti spasmodic medications as drotaverine or others
- GI specific antibiotics as Rifaximin or metronidazole
- probiotics and prebiotics support GI flora great
- bulk forming laxative as Ispaghula husk or Psyllium
- supplements of multi vitamins with minerals, anti oxidants
- regular aerobics
- control of blood sugar
- control of BP, body weight
- cholesterol panel maintenance

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
