Suggest Treatment For Change In Voice And Hoarseness

Medical History Change in voice from the last 4 years By profession i am Assistant Professor, and takes 2 to 3 hour speaking in classes per working day without microphone. I am not diebetic, and usually having normal ranges of blood pressure 70/110. I was gastational diebetic on both of my pragnencies from 3rd timester. I have a very stong family history of diebetics. Both parents, grandparemnts, aunts, uncles, everyone is diebetic. Except me, both of my sisters are diebetic from age 12. I am the only one left. My current weight is 63.0 Kg and heigt is 5 ft 4 inches. Current Age: 32 Gender: Female, married I never in my life smoked or drinked, nor anyone around me, as it is not allowed in our culture and Islam. Sometimes i sleep with mouth open because of my slightly blocked nose. My consultant was ENT surgeon Dr. Zafar Farooqi from Karachi, Pakistan Treatment suggested on 30-07-2013 Syrup Dijex MP, 2 table spoons 1+1+1 Cap. Gocid 40mg 0+0+1 Tab. Phtogenzyn 1+1+1 for 7 days Oxycron nasal spray, 1 puff, 4 times a day Syrup 5% Rhinatrial 1+1+1 (There is a possibility that i may be wrong in spellings of my drugs) He suggested me some tests. Following are the reports: On 31-07-2013 I had a radiology of STD-PARANASAL SINUSES Results: Frontal sinuses are within normal limit. Maxillary antri shows mucosal thickening No fluid level seen, suggest sinusitic changes Mild right sided DNS noted along with thickened turbinates Helicobacter Pylori (IgG) = 1.78 on 01-08-2013 Routine Hematology - Hematology-I Lab Report on 01-08-2013 Haemoglobin = 12.0 g/dl Haematocrit = 37.3% RBC = 4.50 x 10E 12/L MCV = 82.9 fL MCH = 26.7 pg MCHC = 32.2 g/dL WBC = 9.0 x 10E9/L Neutrophils = 65.2% Lymphocytes = 27.5% Eosinophils = 1.0% Monocytes = 6.2% Basophils = 0.1% Platelets = 226 x 10E9/L ESR (Westergren) = 28 mm/1st hr Peripheral Film = Normocytic, Normochromic On 15-08-2013 After analyzing at the reports, my doctor suggested me the following treatment TREATMENT: Avamys Nasal Spray, 2 puff twice daily for a month Tab. Kalamacid 250mg for 2 weeks, 1+0+1 Tab. Estinizole 400mg for 2 weeks 1+1+1 Syp. Dijex MP, two tablespoon, 3 times daily Doplet-3 once / week, for a month After all these three visits, my voice gets better for almost an year but not as perfect as before. In the start of 2016, voice again starts gettin horase I tried myself all possible home remidies, but nothing worked Ayamys nasal spray (because of log use over the years) doesnt work on me anymore to open my blocked nose. I visited my consultant again. He observed physically and suggest me that my throat and nose seems clear this time. He observed my report of thyroid profile. Thyroid Profile: On the date 09-Jul-2016 T3=0.837 T4=7.49 TSH=2.48 I was not sure about this thyroid profile, because of it was made from LUMHS dioginistic and research laboratory. Vitamin B-12 = 317.0 on 09-Jul-2016 Helicobacter Pylori IGG =0.822 on 09-Jul-2016 On 15-07-2016 He suggested me the following treatment Tab. Thyroxine 1+0+0 for 7 days Tarisin nasal spray 2+0+2 for 7 days Cap. Ruling 0+0+1 for 7 days Syp. Mucane 1+1+1 Steam Inhalation with Tinction Benzoin for 10 days at night Tab. Nuberal Forte for 7 days After having thyroxine for a week I personally went for test for thyroid in a reputed laboratory of Pakistan Agakhan Reports on 21-08-2016 Anti Thyroid Anti-Thyrod Peroxidase = Serum Anti Thyroglobulin = Thyroid Profile Serum Total T3 = 1.52 Serum Total T4 = 7.26 Serum TSH = 3.12 No change in voice. It is still the same. I am not continuing any medication now. Voice gets more worse in evening, better in a day, I am not on voice rest. Eating almost everything, but controlled.

As per your history, it may be due to vocal cord inflammation.

For that, I would suggest you do laryngoscopy after ENT consultation.

Kindly give rest to voice. Take a soft and liquid diet.

Do you have a history of weight loss, difficulty swallowing, decreased appetite?

In that case, you may require biopsy after consultation.
