Suggest Treatment For Chest Pain Along With Deep Breathing

my boyfriend has these episodes where he has extreme chest pain. (not super often) ever since they started a few years ago, if he coughs hard or breaths deep it hurts. he is in excellent shape and exercises frequently, and has never had any issues while exercising. sometimes he consumes a lot of sugar and i wonder if that is related. he believes it to be his heart that is hurting, it will hurt for a few seconds and then go away… on and off like that for sometimes 45 minutes or longer. any idea what it could be?

I have gone through your query. Possible cause of your problem can be:
1. Costochondritis- for this pain while palpation on rib area and pain while deep breathing or coughing is characteristic.
2. Cardiac chest pain- As you have pain last for some time and going a ischemic cardiac cause should be ruled out by doing an ECG.
3. Pleuiritic chest pain - this also can give pain while deep breathing and coughing. A chest X ray should be taken to rule out this.
If costochondritis and you were my patient i treat with Gels of anti inflammatory agents like nimusulide for local application. If pain is severe then i advise tab nimusulide orally. If still pain is severe then i advise local injection of corticosteroids or anesthetics.
