Suggest Treatment For Chest Pain In Case Of MI

I am a post myocardial infarction patient. catherisation was done in 1985 after heavy stroke and resucitation and recovery. many tests performed and later Doctors advised to stay on medicines, no need of bipass surgery as the block in the coronery artery(spasm) already cleared and contiuing on treatment with nifidipine, persantin and isordil was told enough. later after ten years Doctors advised to take only tenormin, apirin and digetional tablets like rantidine. later on 2009 I have intermittent chest pain and Doctors advised to take lipitor, nifidipine and persantin along with atenolol. Today after taking medicine I have little pain on chest and I checked my BP and pulse. BP is 140/80 and pulse is 121. Please advise me qhat to do.

hello,I have gone through your query. Thanks for using HCM.
you must first your chest pain with the pain you had at time of infarction. If it is similar to that it is quite likely that it may be angina.
I have gone through record of your medication.i suggest you to get your ECG and Echocardiogram done.Echo will tell you about previous infarction and information on pumping ability of heart.Ecg may show fresh Ischaemia.Depending upon that you may need stress test or angiogram.
You may add Ace group of medicine after discussing with your treating doctor.
My best wishes

Brief:
Work up to be done if your doctor thinks is cardiac.

Detailed:

Hello,

Thanks for Writing to Health Care Magic and putting trust in us regarding your Health related query.
I am Dr. Saleha and I have read your message in detail, I understand your concern and will try to help you in this.

I am pleased to know that you are doing fine since 30 years , there are two types of chest pains which have to be told apart by examination and investigations.

I would suggest you to meet your doctor and your doctor will decide if these pains are heart related or chest wall related, it might also be your stomach doing it.

If you doctor gets an impression that it's heart related he might go for ECG and Exercise tolerance test with heart enzymes , these things are usually diagnostic. Then further investigations like stress echo and even angiography can be done (if your doctor has a high suspicion and initial investigations also suggest cardiac issue.

I would suggest you to meet your doctor as soon as possible, Bp of 140/80 or pulse or 121 are quite non specific , pulse can be raised in setting of pain and anxiety of a cardiac disorder.

Till then keep using these medications and keep an eye on your resting pulse rate, don't panic everything should be fine.

I hope this answers your question. If you have more questions, I am happy to answer.
Wish you lasting health.

Regards.

Dr.Saleha Saeed.
