Suggest Treatment For Chronic HBV When SGOT Level Shows 70

Hi Dr. I am suffering from Chronic HBV for the last 5 years and since then i have been under the treatment of Baraclode, I do my tests every six month and my results have been normal for the last 3 years, however recently when i did my test 2 weeks ago my Bilirubin was 3.8 SGP was 142 and SGOT was 70, all other tests, like Ultrasound, PCR, FIbroScan were normal and my Dr. didnt say a word about the elevated bilirubin, SGPT and SGOT, can you please tell me how serious these can be in my case? Thanks. ZM

Dear
Thanks for raising your concern on this platform and after reviewing your history I'm of the following opinion:

1. Since HBV is chronic (6 years) thus it is Chronic Hepatitis I.e. chronic persistent hepatitis type.

2. High bilirubin and SGOT/SGPT level might indicate ongoing inflammation and since are high recently thus use of alcohol, pain killers or any acute hepatic infection has to be excluded.

3. One has to specifically look out for hepatocellular failure in which all three are raised but additional Prothrombin Test (PT) is done to know the extent of hepatocellular damage (if there)

4. Since there is no specific treatment,so
. Bed rest with bathroom privileges
. Low fat, high carbohydrate diet
. Boiled water to drink
. Booster dose of Hepatitis B can be considered
