Suggest Treatment For Cyst In My Epididimus

Need advice. In the remote past, I was told by a urologist that it was likely that I had a cyst in my epididimus. I am a 43yo male. That was years ago around 15. Since that time, My right testicle has been very painful to the touch especially during sex. Lately, my symptoms have worsened. On self examination, the epididimus seems more swollen. I now have some pain at rest and cannot tolerate my testicle being touched during sex. Sometimes the situation seems better after ejaculation. In addition, I believe that I can palpate a swollen lymph node in my groin. I would like to know who I should see about this, what diagnostic/imaging tools can be done, and how this problem is usually treated (I am assuming surgery of some type). Thank you, david

Hi! this is very common and in fact every alternate day I get one such case; don't worry whatever the duration!

And thanks for giving us an opportunity to discuss on epididymal nodules/groin lymph nodes.

I usually do a needle aspiration biopsy under local anesthesia if needed and pass a needle under ultrasound guidance and generally get slightly turbid whitish fluid in 90 % of cases that proves to a spermatocele or epididymal simple retention cyst. Rarely it yields pus suggesting abscess/tuberculosis.

In rest of the cases we get blood-mixed tissue fragments that proves to be adenomatoid tumor, a benign lesion; stromal nodule, old trauma/related reaction, tuberculosis or just infection! Malignancy we get rarely less than 1% cases!

It could be related to swelling, pain, infertility, testicular tumor etc and might drain its infection to groin lymph nodes.
