Suggest Treatment For Diabetes

I have been on 14units of Lantus and 2000mg of Metformin daily, which have successfully controlled my type 2 for two years. In Jan, I got sick with the flu and have lost all control of my numbers. Fasting glucose is 220 and post pradial ranges from 280 and up. The numbers don t make sense. They are high even with a meal with zero carbs. Should I raise my dosage of Lantus? Do I need a fast acting insulin?

Hello,

Yes, your sugars are to be controlled. My recommendations are as follows:
1) Fasting glucose levels are to be controlled first. Lantus 14 units has not been enough, so we need to increase the Lantus dose to 22 units once daily.
2) Metformin 2000mg is to be continued
3) You need a rapid acting insulin only before a major meal
4) You can take Apidra 6units which is a fast acting insulin only before a major meal
5) Continue the above treatment for 1 month and then get tested the sugar levels
6) You can update after getting tested
7) In the meantime, if you experience hypoglycemia or lowered sugar symptoms like sweating, rapid heart rate, dizziness, you can decrease the dose of Lantus by 2 units
