Suggest Treatment For Diabetes

I AM SUFFERING FROM DIABETES TYPE 2 SINCE 1995.CONTROLLED UP TO 2003-2004 ON MEDICINE. THEN I WAS ON GLYREE4 TWICE DAILY ALONG WITH PIOMED15 ECOSPIRIN CARDIOPRIL2.5 BUT UPS AND DOWN WERE THERE.NOW ON GLUCONORMG2 FORTE (OD), GLIZIDM AND VOBOSE.3(BOTH TWICE DAILY). BUT FBS IS 271 AND PP IS 248.ALBUMIN CREATININE RATIO IS 75.86, CREATININE IS .9, CHOLESTEROL NORMAL, HBAC 9.FEELING WEAK. AGE-48 WT-85KG.A.K.GUPTA, GURGAON

As per the history, you have been suffering from Type 2 Diabetes mellitus since 1995 (almost 20 yrs), which was controlled in the initial 8-10 yrs with oral medications.
Presently you are on Gluconorm G2 forte (Glimepiride 2mg and metformin 1000mg) once a day, along with Glizid M (Gliclazide 80mg and metformin 500mg) and Vobose (Voglibose 0.3mg), twice a day. Despite multiple medications, your blood sugar levels are not adequately controlled, as evident by your HbAic level of 9%, FBS OF 271mg% and PPBS of 248mg%. Under such circumstances it is advisable that you can start a new group of drugs called the DPPIV inhibitors (like vildagliptin) along with your present regimen, or you can switch over to injectables (like Insulin or Exenatide-GLP1 analogues). However, taking you history into account, Insulin (either basal-bolus regimen or premixed insulin regimen) would be ideal for you.
You have mentioned your weight to be 85kgs, however, to calculate your BMI, your height is also needed. If you are overweight, it is preferable to reduce your weight and maintain your BMI If your ACR is 75.86, a regimen of ACE inhibitors or ARB is recommended. Diabetic subjects are proned to suffer from neuropathy and metformin itself can lead to Vitamin B12 deficiency, which may cause weakness. Hence, vitamin B complex supplements are required.
I would advise you to visit an endocrinologist, who can give you an outline of management of your Diabetic status.
