Suggest Treatment For Disc Bulge At L4-L5 Level Of Spine

I just received my mri results which sa there is a moderate disc protrusion at L5-S1 with an annular tear. I have been going to physical therapy for 8 month because the last mri results said that discs L4-L5-S1 had buldges. This has gotton worse, will I need surgery?
