Suggest Treatment For Dizziness And Weight Gain Despite Taking Thyroid Medication

I was first diagnosed with hypothyroidism in 1955. I was originally given Armour thyroid supplement. I might as well have taken nothing. Along in the late 1960 s, I was given Synthroid. The difference was like night and day! Then they started increasing my dosage. Finally I was taking what I recall as a blue round barrel shaped pill, but I don t remember the dosage. But with that, I went back to 9th grade, and with 3 teenagers and 5 five-year-olds at home, I was able to go to college and get my degree. I am female, 5 4 , and averaged 170lbs. all those years. After having surgery for hiatal hernia in 2011, I dropped to 115 lbs. Now, at age 74, they ve been decreasing my dosage gradually for several years. I had gained back to 140 lbs., but I have been sick this last 6 months or so, they said a urinary infection, treated me 6 times for that before admitting that only once had I tested positive for that. I ve had dizzy spells that I would classify as seizures, forgetfulness, even amnesia, feel weak and tired, and am down to 120 lbs. now. While this is going on my thyroid dosage has been lowered twice, is now at 0.100MG, and the latest test, they said that my thyroid is low, so it needs to be lowered further. I asked if it is low, why does it need to be lowered further, and it has been explained to me 3 times that low really means high, so it needs to be lowered further. This makes no sense to me. Can you explain it so that I can understand?

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

You actually have some symptoms (tiredness, forgetfulness, amnesia) that can be due to hypothyroidism. Anyway when a patient with hypothyroidism is under thyroid replacement therapy (synthroid) it is necessary to do the TSH test to understand the actual thyroid situation. At the other side you mention that your treating doctor has lowered your synthroid dosage because your thyroid is low. In my opinion the exact explanation would be that your TSH test is low. This means that you are in circumstances of hyperthyroidism or Iatrogenic hyperthyroidism and this means that you take too much thyroid hormone replacement therapy for what your actual body needs. There is a negative feedback that control the production of TSH from the pituitary gland and the thyroid hormones T3 and T4. So, lower TSH means high production of the thyroid hormones from the thyroid gland or hyperthyroidism. This is why low TSH test means hyperthyroidism. In your circumstances is the result of overdosing your synthroid and decreasing the dose of synthroid is the right thing to do.

Hope I have answered your query. Let me know if I can assist you further.

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

You mention that your doctors said that your thyroid is low, so it is necessary to lower the synthroid dosage further. When a hypothyroid person take synthroid it is necessary to do the TSH test to monitor if he need dose adjustment. So, I understand that your TSH test may be low and for this reason your treating doctor has lowered to dosage of thyroid hormone replacement therapy. If TSH is low or suppressed this means hyperthyroidism or in your circumstances this is the result of high doses of synthroid for your body needs. This is the explanation why low TSH test means high or hyperthyroidism. Don't worry the lower dose is the right treatment for you.

Hope I have answered your query. Let me know if I can assist you further.
