Suggest Treatment For Ear Infection And Sore Throat

My 6 year old daughter with Down Syndrome was diagnosed 6 days ago with ear infection. She was treated with ciprodex and bactrim. While being treated she developed a terrible sore throat, high fevers, refusal to eat, trouble drinking. A rash appeared on the 4th day. On the fifth day, the Pediatric PA diagnosed her with strep/scarlet fever. She is now being treated with azithromiacin(max)? Her rash is worsening. Fevers are lessening in intensity. Drinking and some eating has resumed. Should I worry about the increased rash symptom? Is there a time when I should take her to er even if she isn t running high fever, simply because of the rash? Also, the rash looks identical to scarlet fever rash photos on the internet. Thanks, Chris
