Suggest Treatment For High Fever, Rashes On Arm, Leg And Foot

Uncle admitted 2 days ago high fever confused Rash on arm leg foot blood work only showed infection 104 fever they started antibiotics fever down to be dismissed than Blood pressure crashed respiration labored in ICU no diagnosis he has large abdominal hernia I think it is the cause sanguine us ? Concerned he is failing fast don t know what to do m .d Doesn t have any amswers

Hi,

It is painful to read about your uncle.

Your uncle has severe form of infection with shock.
The infection may be -
- Bacterial infection.
- Viral infection like Dengue fever with shock.
- Parasitic infection like malaria.

There is a need for-
- Investigation to know underlying cause and effect of infection on various organs of body.
- Antibiotic treatment with broad spectrum antibiotics.
- Maitainace of adequate blood pressure for body perfusion.
It may require inotropic support.
- Monitoring of vital organ function: hourly monitoring of urine out put, Heart rate, blood pressure, blood sugar, bleeding, level of consciousness.

In such cases following investigations may be advisable -
- Complete Blood Count.
- Renal function test - urea, creatinine, electrolytes.
- Liver function test.
- Blood culture and sensitivity testing.
- Urine examination including culture and sensitivity testing.
- Dengue NS 1, IgG & IgM.
- Peripheral Smear For Malerial Parasite.
- Leptospira IgG & IgM.
- Ultrasonography of abdomen and pelvis.
- Coagulation profile.
- Serum Procalcitinin.
- Chest X-ray.
Depending upon clinical evaluation and above test results there may be need for -
- Brain imaging like CT in case of suspected meningitis.
- CT scan of abdomen and pelvis or Thorax in case of suspected abdominal (urinary or gastrointestinal sepsis) or lung infections .
- CSF study in case of suspected meningitis.
