Suggest Treatment For High Grade Fever And Vomiting

My son, 10 years old, had a high grade fever 104, with vommiting for over 24 hours . Two days ago he had mental status change for about 10 minutes . He was confused, slurred speech, did not even recognize me. We immediately called 911 and lowered his body temp via shower. Within 5 minutes his mental status returned to baseline. The ER, did blood work, and urinalysis, IV fluids, but because his mental status returned did not tap him or do any further testing for encephalitis. The fever now 2 days later is fluctuating between normal and 101. Should we be requesting further testing to rule out viral encephalitis or just assume altered mental status was do to fever/ slight dehydration ?

General & Family Physician 's  Response
Hello,

I understand your concern. It was good that he was treated immediately in ER. The cause for his condition can be viral or bacterial or some other infection. Although till the previous test results come up, let his condition improve. The altered mental status can be due to high grade fever or due to the infectious cause.

I suggest you keep an eye on his body temperature and note down all the fluctuations. It will take time to go down completely. I assume the doctors have prescribed medicine before discharging him, so make sure those are given on time. If the fever rises back to 103/104 degree C despite giving medication and similar issue happens including vomiting then it’s better to take him to ER and talk to the treating doctor regarding further testing because the doctors must be waiting for the previous test results, depending on that if required they will suggest further tests.

Take care. Hope I have answered your question. Let me know if I can assist you further.
