You complained of severe itching on arms and back. And that is more at night. You did not tell about other symptom or disease. You do not have any skin lesions. It means that some internal problem may be there. You consult dermatologist for firm diagnosis. He might suggest blood investigations to detect any internal disease. Anaemia,DM,HIV,thyroid dysfunction, renal diseases, liver dysfunction..etc may be ruled out.
Dryness or xerosis is another cause responsible for the itching. You may take antihistaminics like levocetirizine till the relief in pruritus. If needed, steroid may be taken in tappering dose. Liq.parraffin containing cream may be used to reduce dryness. Soap bath should be avoided. If internal cause is found, have treatment by physician. Mild steroid cream may be helpful. Have patience. I hope you got my answer.
Thanks.
