Suggest Treatment For Itchy Foreskin

The foreskin around the tip of my penis has been itchy, there has been redness (may be a result of scratching) and is somewhat tight to pull back, and when I pull it back, there is a vein in the foreskin showing itself very prominently on the left side near the head. What s wrong?
