Suggest Treatment For Lethargy,unsteady Gait And Confusion

My 68 yr old mother suffers from COPD/Emphysema and is on Oxygen She is under pain management as well and also suffers from depression. For the last 2 weeks she has been lethargic , walking unsteady , and confused. She no longer smokes and i mange her medications for proper dosing. Her pain management is 20-30 mg of methadone and her anti depressant is 150 mg of effexor as well as some heart meds and nebulizers. I want to call 911 as she is not the same...i am disabled as well and cannot drive. Please help
