Hi,

If such aggressive therapy isn't working properly, you may need to have fresh MRI of lumbar spine and cervical spine and get a neurosurgeon review for any nerve impingement and options of surgical intervention like laminectomy or repair with prosthetics etc. Pain medicine specialist review for nerve blocks may also be needed.

Take care. Hope I have answered your question. Let me know if I can assist you further.
