I appreciate your concern for the presence of detectable hCG in your semen analysis with positive pregnancy test immediately following the sexual activity.
After having gone through the problem described in your query I am of the opinion that you seem to be having the testicular lump as a cancer referred to as
germ cell tumor or choriocarcinoma.
It produces
human chorionic gonadotropin (the main hormone actually produced by the placenta during pregnancy) which is secreted in the semen and hence the home pregnancy test is positive without having actual pregnancy.
It also produces a tumor marker called as
Alpha-fetoprotein (AFP)
The raised enzymes like LDH & AST are also associated with the tumor which may indicate the spread of the disease to the liver until proved other wise.
In view of the above explanation, I would advise you to consult an
oncologist for proper evaluation and to conduct additional lab tests such as
Alpha fetoprotein, CT scan, Liver function tests to confirm the diagnosis.
The standard treatment for testicular choriocarcinoma include 4 cycles of chemotherapy (Bleomycin+Etoposide+Cisplatin).
