Mediastinitis is very rare and life threatening condition. The common causes are trauma, fungal infection (histoplasma) etc. Sometimes anaerobic infections and post operative infections after cardiac surgery can also cause mediastinitis. The treatment should include antifungal drug with anaerobic coverage like metronidazole or clindamycin and broad spectrum antibiotics like levoflixacin, imipenem etc. In my opinion consult CVTS surgeon for any surgical cure. And consult pulmonologist for pulmonary embolism issue and titrate dose of warf.
