Suggest Treatment For Mood Swings And Postpartum Depression

Q: My wife gave birth to our son approximately 4 months ago, she demonstrated signs of postpartum depression immediately and I tried hard to keep her environment as stress free as possible and upbeat. At times she seemed to be doing better, then a week or two later she would fall back into the depression. Lately though she ha sincreased the frequency of her mood swings. She will be very excitable one minute then very angry the next just from a disagreement. She has been spending money irrationally and has picked up a drug habit. She has this constant fear that Im going to leave her or that Im cheating, which Im not doing either of those. Today she actually got violent with me while I was holding our son. My gut instinct tells me that I need to have her looked at immediatley, but I know how unstable she is and any kind of confrontation would become violent, I dont want to be the enemy to her but I know she would view it that way. What do I do? Symptoms: Irritability, irrational thinking, violent outburst, drug addiction, family history of Bi-Polar Disorder

Hello,

You have mentioned that you have postpartum depression, but the depressive symptoms are present since teenage years. These symptoms started after some embarrassing moment and still, the horrible feelings are persistent. The feelings of guilt are there. For these persistent symptoms, in my opinion, you should consult a psychiatrist. As the postpartum depression harms both mother and baby so should be treated with proper attention.

Medicines like SSRIs especially Sertraline or Fluoxetine can be used. Sertraline is even a safe drug during breastfeeding also. These drugs will help to reduce unwanted thoughts about past incidents and will improve your mood. Please consult a psychiatrist for evaluation and treatment.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein, General Surgeon
