Suggest Treatment For Neck Pain Travelling Down To The Deltoid

I have pain which seems to come from my neck into my right shoulder travelling down to deltoid. Also pain going down into shoulder blade. This started around the beginning of March gradually getting worse in spite of chiropractic, physio and massage. I now get tingling in upper arm, sometimes going down to my wrist. My finger tips feel a but numb. I have good range of motion but get pain on coughing, laughing, walking, blowing my nose and any kind of straining. Very tender spots back of neck, top of shoulder, deltoid and upper shoulder blade where muscles attach.
