Hi,

Noted your short history about hysterectomy 4 weeks ago, bleeding started yesterday, spotting today and have lot of pain. The possible causes are:
Granuloma that develops at the internal stitches.
Infection at the operation site.

I would advise you the following:
Consult your Operating Gynecologist asap.
Proper examination will tell us the cause.
This may need just cauterization or so.
Antibiotic cover may be needed as well as local hygiene is to be maintained well.
Do not try anything at home.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
