Suggest Treatment For Pain And Stiffness In Shoulder Joint

I had an injury 12 months ago where i was accidently head butted in the back by a small child at the nursery i worked at. As a result of this, i suffered back pain but also severe shoulder and neck pain, i saw a doctor and was referred to physio. i was also given diclofenac. Since then, i have had my second child and the last couple of weeks, my shoulder has got worse, It feels tight all the time and i am unable to loosen it up. My husband is working away as he is in the Army and i am unable to get frequent massages from family as we live in Germany due to his job. I was wondering why i am still having problems a year on and what can i do to help.
