Suggest Treatment For Pain Due To Paracentral Disc Extrusion At L4 L5 Level

Hi i am 44 yrs old male and suffering from low back pain with radiating to right calf muscle and hypoaesthesia in rt. great toe for 70 days. In MRI , there is right paracentral disc extrusion at L4-L5 level. I took bed rest and NSAID but no significant improvement. What is the treatment of choice for my problem?
