My 3 year old son complained about penis pain around 6pm. We looked and when the skin was pulled down it was slightly red so we put some neosporin with pain relief on it. At 1030 pm I checked it again and half the foreskin was extremely red and swollen now at 1130 the entire perimeter of foreskin near the head is swollen. Should he go to the er

* As per my surgical experience, the severe pain in penis with a swollen foreskin can be due to
- paraphimosis
- acute balanitis
- other infection

* This sounds as a surgical emergency and you must take him to ER without any delay.
