Suggest Treatment For Pain In The Perineum And Scrotal Area

Male 55 with pain in left perineum and scrotal area. Worse at night and wakes me. Feels like kicked in groin on left side. Went to ER and did Cat Scan and said no hernia but said Acute prostatitis. Been to urologist 3 times and they think prostatitis but not seeming to improve. Think they are out of options for me. Weird as on left side and have had prostatis before but did not feel like this. I did have left testiical removed for a spermatocel 5 years ago. Had inguinal hernia repairs 29 years ago. Saw surgeon to see if hernia and they said no. Treatment has been antibiotics and anti inflammatories. Still hurting though after 4 weeks of treatment. I am do for my 10 year colonoscopy so going to get that. The waking up hurting and clammy is no fun. Any ideas what to do next?

Get a USG done, go for PSA test (prostate specific antigen) and urine routine and culture. Let your doctor start antibiotics course according to your culture report.
