General & Family Physician 's  Response
Hello dear
Understand your concern.
Exact diagnosis need examination and proper investigations.

Following are the causes of pain in testis
Trauma, Orchitis, Varicocele, hydrocele, Cyst of epididymis, Cancer of testis.
You should consult the urologist and do following test:
Blood: CBC, ESR, Colour dopper or USG of scrotum, Biopsy should be take from testis if needed, Urine routine and microscopic examination
That help in diagnosis and then take treatment after proper diagnosis

In the mean while take
Cefexine and Diclofenac twice daily, apply warm pad, support the scrotum, wear loose undergarments.
Hope this may help you
Thanks and Regards.
