Suggest Treatment For Painful Lump In Vaginal Area

Hi, I have a large lump on the side of my vaginal area. It's very painful. Two and a Half years ago my dr. Said not have have it removed. It wasn't painful then, now it's red and it hurts a lot and I don't have insurance. What type of dr. Can take care of this without surgery? CAn I sees determologiist or does it have to be an obgyn?
