Suggest Treatment For Panic Attack In An Elderly Person

I am a female 74 years of age. Last Saturday I went to dinner with friends, had a glass of wine (which I do not usually drink). Early Sunday morning (3:00 a.m.), after a restless night, I had a compression feeling in my chest and both upper arms. Have not had any further symptoms. Could this be panic attack instead of heart related. No heart condition that I know of at present.

Thank you for your query. I understand your concerns.

From what you have mentioned, it is difficult to say whether it is due to a panic attack or due to heart problem as the symptoms for both of them are almost similar. Anxiety/panic attack can lead to chest tightness or pain. Angina which is due to narrowing of blood vessels of the heart can also cause chest pain and radiation of the pain to the arms. The arm/arms can be numb too.

Were you asleep during the episode or were you awake?

Was there any trigger factors for the episode? Like for e.g. were you stressed, or were you thinking and worrying about something prior? Stress and worry can lead to anxiety and panic attack. Anxiety by itself is a trigger factor for cardiac related angina/chest pain.

Did you have any other symptoms like palpitations (fast heart beat), tingling in your fingers and around lips? Was there nausea? These symptoms are related to panic attack.

Palpitations can also occur in angina, so can nausea.

So, as you see, just based on symptoms alone, it is difficult to distinguish if chest pain is due to heart related reason or due to anxiety.

Even though you do not have any other medical problems, as a person ages, there is a small risk of blocks forming in the blood vessels.

Hence, I would advise you to see you doctor who would do a thorough assessment of you. He would run some basic blood tests like blood glucose, lipid profile, kidney function tests, Thyroid function tests and also do an ECG. Based on these results, your doctor may refer you for a 2D ECHO and a TMT.
